Blackfoot Confederacy COVID-19 Experiences and Impact Survey
The Blackfoot Confederacy Tribal Council is partnering with the Health Quality Council of Alberta to survey the people of the Confederacy Nations on public health measures, health system access, vaccination, as well as the support available to stay informed, well and safe during the COVID-19 pandemic.

The survey should only take about 15 minutes to complete. All Blackfoot Confederacy people over the age of 16 are invited to participate. All responses are completely confidential.
Help the people in your life participate
If you know someone who may have difficulty accessing or using a computer, open the survey via the link above, give them a call, and go through all the questions, inputting their answers into the online survey on their behalf. We need your help to prepare for the ongoing pandemic response, and every voice counts.

What happens with the information we collect?
